Before your camper arrives at Camp Manitowa, we must receive all required health information and documentation.

Required Forms

All campers must submit the following forms before attending camp:

Download ACA Camper Health History Form

Provides important information about your camper's health history, allergies, medications, and any physical, emotional, or learning needs.

Download ACA Camper Healthcare Recommendations Form

Completed by a licensed medical professional and confirms your camper's readiness to participate in camp activities.

All required forms must be received no later than June 1.
